Prizes of $600 to $250,000 can be claimed at any Florida Lottery District office or Florida Lottery Headquarters in Tallahassee.
Prizes of $600 or more require the completion of a Florida Lottery Winner Claim Form and an Internal Revenue Service Form W-9 “Request for Taxpayer Identification Number and Certification” form.
Prizes greater than $250,000 must be claimed at Florida Lottery Headquarters in Tallahassee.
