How to claim Holiday MILLIONAIRE RAFFLE$10,000 and $100,000 Prizes
Each Holiday MILLIONAIRE RAFFLE drawing winner must submit his/her official Holiday MILLIONAIRE RAFFLE ticket, a completed Florida Lottery Winner Claim Form and IRS W-9 Form,
and a copy of an acceptable form of identification for payment at a local Florida Lottery office.

$1 Million Prizes
Prizes of $1 Million must be submitted for payment at Florida Lottery
Headquarters in Tallahassee. Players should call Customer Service at
(850) 487-7787 or TDD (850) 487-7784 for instructions and assistance.
Holiday MILLIONAIRE RAFFLE tickets are the only valid receipts to redeem
Holiday MILLIONAIRE RAFFLE prizes. Players can validate their tickets beginning
January 2, 2007. Players must present their winning ticket for validation
at a Lottery office or retailer within 180 days from the date of the
drawing (June 30, 2007). If the prize is not paid at the time of validation,
the player must submit the winning ticket for payment at the respective Florida
Lottery office, as required by the Lottery's rule governing payment of
prizes (A.C. 53ER06-4). Failure of a prizewinner to claim a
Holiday MILLIONAIRE RAFFLE prize by submitting the winning ticket for
validation and payment in accordance with the Lottery's rule A.C. 53ER06-4 shall
result in forfeiture of the prize.
If a winning Holiday MILLIONAIRE RAFFLE ticket bearing any of the first ten winning numbers
is determined to be forfeited, the 11th through 30th prizewinners who have submitted their
tickets for validation and payment in accordance with Lottery rule 53ER06-4 will be used
in the order in which they were drawn to select alternate winners for the $1 Million
top prize. The alternate winner will be awarded the difference between the $100,000 prize
and the $1 Million prize.
If applicable, the Florida Lottery will attempt to notify, for a period of two weeks, the first
alternate drawn in the $100,000 prize category, who will be awarded the higher prize. If the
Florida Lottery is unable to contact the first alternate, the Florida Lottery will attempt to
notify, for a period of two weeks, the second alternate drawn. This process will continue until
an alternate is contacted or the Florida Lottery has exhausted the list of available
alternates, in which case the $1 Million prize will not be awarded.
Payment of all federal, state and/or local taxes will be the responsibility of the winner.
Federal withholding taxes will be deducted from the cash payment.
