How to Claim

If you are a winner in the 25th Anniversary MILLIONAIRE RAFFLE, congratulations!

Prizes of $1,000, $10,000 or $100,000 may be claimed at any Florida Lottery office and prizes of $1 million or more must be claimed at Florida Lottery headquarters in Tallahassee.

If you claim a $1,000 prize from a weekly drawing before December 31, 2012, you will be issued a continuation ticket for the Grand Prize drawing. Hold onto your continuation ticket as it will be automatically entered into the Grand Prize drawing and you must have the ticket to claim a prize.

All 25th Anniversary MILLIONAIRE RAFFLE winners must present their original 25th Anniversary MILLIONAIRE RAFFLE tickets, a completed Winner Claim Form and an acceptable form of identification to a Lottery office for payment.

Acceptable forms of identification include:

• An identification card or driver's license issued by a public agency authorized to issue driver's licenses in Florida, a state other than Florida, a territory of the U.S., Canada, or Mexico.
• A passport issued by the U.S. Department of State.
• A passport issued by a foreign government.
• An identification card issued by any branch of the U.S. Armed Forces.
• An identification card issued by the United States Bureau of Citizenship and Immigration Services.

25th Anniversary MILLIONAIRE RAFFLE tickets of less than $1 million must be claimed within 180 days of the winning draw date. Winning 25th Anniversary MILLIONAIRE RAFFLE tickets of less than $1 million may be mailed for redemption to: Florida Lottery, Claims Processing, 250 Marriott Drive, Tallahassee, FL 32399-9939. The risk of mailing tickets remains with the player.
